1. All fees required for enrolment MUST BE successfully paid before the start of the class/activity, otherwise JC Sports reserves the right to deem the application has been given up by the applicant.  JC Sports also reserves the right to deny the students attending classes should there be any unpaid fees.
   JC Sports保留權利視申請人為已放棄報名。如果有任何未付費用，JC Sports亦有權拒絕未繳清費用之學員上課。
    

2. Once the course fee is accepted, course fee is non-refundable, cannot be rescheduled & non-transferable under any circumstances.
   課程費用一經繳付，於任何情況下恕不退回、更改或轉讓。
    

3. No leave will be allowed.  No make-up class or refund of fees will be arranged for absences.
   不設請假安排。如自行缺課，恕不補堂或退款。
    

4. If the classes/activities are cancelled due to typhoon, thunderstorm, rainstorm or any bad weather condition, JC Sports Development Limited (“JC Sports”) will inform the Players via SMS and/or mobile app notification one (1) hour before the classes/activities.  We will notify the Players the make-up arrangement.
   如因颱風、雷暴或暴雨而取消該節課堂或活動，JC Sports Development Limited (“JC Sports”) 會於課堂或活動開始前一小時以短訊或手機應用程式通知學員。有關補堂安排會另行通知。
    

5. In case of light rain condition, the classes/activities may still go on as scheduled.  The coach will consider the actual weather condition to decide whether or not to proceed with the classes/activities.  If the classes/activities are cancelled after commencement due to bad weather condition, no make-up classes or refund will be provided.
   在微雨情況下會照常上課。教練會因應實際天氣情況決定是否繼續上課/活動，
   如課堂中途因天氣惡劣而教練決定取消課堂，恕不設補堂或退款。
    

6. Make-up class will be arranged by JC Sports in the case of class cancellation due to venue availability and/or unexpected events. We will notify the Players the make-up arrangement. We reserve the right to refuse any player, who are not eligible, to participate in any make up training sessions.
   如因場地安排或其他突發事件而取消課堂，JC Sports將會安排補課。補課日期及時間將會另行通知。JC Sports保留拒絕任何不符合資格的學員參加任何補課的權利。
    

7. The Players and/or Parents/Guardians of the Players (as the case may be) agree and authorize JC Sports to use the Players’ portrait, name, voice and personal information for the purpose of advertising and promoting of JC Sports’ business.
   學員及/或學員家長/監護人 (視情況而定) 在此同意並授權JC Sports使用學員的頭像、姓名、聲音和個人信息，用於JC Sports業務的宣傳和推廣。
    

8. JC Sports and its officers, directors, service providers and/or employees shall not be liable in any way whatsoever (whether by way of contract, tort, negligence, statutory, common law duty or otherwise) for any personal injury, loss and damage or death which the Players may suffer in the classes or activities.  The Players may purchase their own insurance to cover any personal injury, loss and damage or death according to their needs.  The Players and/or Parents/Guardians of the Players (as the case may be) undertakes to indemnify JC Sports against all actions, claims and demands by any person who suffers or sustains any personal injury, loss and damage or death arising out of or as a result of the above Player’s acts.
   JC Sports及其管理人員、董事、服務提供者及/或員工不需為學員在課程/活動中遭受的任何傷害、損失或死亡負責（不論是通過合同、侵權、疏忽、成文法、普通法義務或其他方式）。 學員可以根據自己的需要購買自己的保險，以涵蓋任何人身傷害、損失和損壞或死亡。如學員的行為而導致第三者受到任何損失、傷害、受傷或死亡, 該學員及/或學員家長/監護人 (視情況而定)須承擔JC Sports所面對的任何索償及要求。 
    

9. The information provided by the Players will only be used for the enrolment and promotion of recreation and sports activities organized by JC Sports and co-organizing parties. For correction of or access to personal data after submission of this Form, please contact JC Sports directly.
   學員所提供的資料只用於JC Sports或合辦機構的活動報名事宜及活動宣傳之用。在遞交申請表後, 如欲更改或查詢個人資料, 可與JC Sports聯絡。
    

10. In case of any dispute, JC Sports reserves the right of final decision. In the event of any discrepancy between the English and Chinese versions of these Terms & Conditions, the English version shall prevail.
    如有任何爭議， JC Sports將保留最終決定權。如本條款及細則的中、英文版本有所差異，一概以英文版本為準。
    
    Declaration : I, the above Player or Parent/Guardian of the above Player (as the case may be) hereby declare that the Player is healthy, physically fit and suitable to participate in the soccer classes/activities organized by JC Sports at my own risk of the above Player and agree to abide by all rules and regulations of the JC Sports.  I further agree that JC Sports and /or its responsible officer(s) shall not be liable for any injury /death and /or loss of belongings/properties that the above Player may suffer in the classes/activities.  I also confirm that I have read and agreed to all the above terms and conditions.
    聲明 : 本人，上述學員或上述學員家長/監護人 (視情況而定)，在此聲明上述學員健康及體能良好及適宜參加由JC Sports舉辦之足球課程/活動及承擔有關風險，並同意遵守JC Sports的所有規則和規定。本人同意如上述學員因參加有關課程/活動而引致受傷/死亡或財物損失，JC Sports及/或其主管人員無需負任何責任。本人在此亦確認本人已經細閱並同意上述所有條款及細則。
